• Eligibility Requirements:Ages 18 years and older with a serious psychiatric condition, history of repeated psychiatric hospitalizations or emergency psychiatric interventions, are homeless or at-risk of homelessness in South Bay, San Diego, and who may also have a co-occurring disorder.. Birth Certificate;SS Card. Please provide Social Security number and birth certificate if available. Referrals may be initiated by a social service provider/professional or a concerned family member by calling the program directly or by fax.
