Program is an Assertive Community Treatment (ACT) model providing services to individuals in the community that include intensive case management, psychiatric care, vocational services, and integrated services for mental health & substance abuse issues.

Referrals may be initiated by a social service provider/professional or a concerned family member by calling the program directly or by fax.
